To save time on the day of your appointment, please download the following forms, fill them out and bring them with you.

Medical Records Retention Policy

Effective immediately we will implement the following medical records retention policy:

  • We will maintain patient records for 6 years following the last patient encounter with the exception of minor children. This will include charts of patients that are deceased.
  • Records for minor children shall be maintained until the child reaches the age of 18, with a minimum time of retention being six years from the last patient encounter regardless of the age of the child.
